I am adding my wife's name to the deed to our home. My question is how to properly fill in the grantor/grantee on the quitclaim deed

Use Mr. Field's answer if you want the property to pass automatically to the surviving spouse.

If you wish to convey a one-half interest to your wife, you should replace the language "joint tenants with right of survivorship" with "tenants in common".

Since you do not explain why you wish to do this, I cannot advise you which option you should choose. You should consult an attorney on this point.

The Grantor would be you, as your name appears on your current deed. The Grantee would be "you and Mary Jane Jones, as joint tenants with rights of suvrivorship"
