Legal Question in Family Law in Illinois

My ex-wife and I have come to an agreement to terminate the monthly maintenance that I pay her. Can I just get that in writing from her and file that as a matter of record on the case (and is there a form for that?) or do I need to file it as a motion and have it presented before a judge to be signed off on? I'd like to do this myself if possible so if I can just file a modification to our settlement agreement or a revised settlement agreement that would be what I want to do. If someone could point me in the right direction I think that would help.
