Legal Question in Employment Law in Illinois

I work for a Home Health Company and I have a non solicitation/non disclosure agreement that is rock solid. The Agrement states I can not contact my current clents for a period of one year.

What happens if a client knows where Im going and sends referrals to me, even though I'm not making contact with them. Can I be sued by my former employer for violating the agreement?
