Legal Question in Family Law in Indiana

I currently have sole custody of my 2 boys which I received after their mom got sent to prison for 3 yrs for drugs. She was out for 2 1/2 years in which she got to see them 4 days a week as a work a 4 on and 4 off schedule. She stayed clean and sober for this whole time and recently relapsed. I am currently only letting her have supervised visits. My question is and this may give her even more incentive to not have another relapse, If I was to draw up a document stating that if she did have another relapse under any circumstances that she would give up lega rights to her children and she was to sign this document and it was notorized would it be a legal binding document.
