Legal Question in Real Estate Law in Indiana

Protection

Before I pass on, I want to make sure my estate does not fall in to the hands of my state ( Indiana ) For example, if I am commited to a nursing home under local Medicaid rules, I want to make sure the state can not confiscate my home & property before or after death. What can I do to keep my property intact with my family, Spouse and Son?

Thank you
