A local attorney forged my medical release. He significantly altered the original form which I signed. My question is when he altered the original form, did this cancel out the original release? It appears he actually cut my signature out of the original document and pasted it on two subsequent forms (same form number) but with significant changes. He made it appear that I signed the release on a later date than I really signed it. I can also connect this to his huge client that had motive to have the date changed to coincide with some of their paperwork to cover up a situation that happened at their facility. After he knew I found out about the forgery, he did try to get me to sign a new release; but I did not do so. To make matters worse, since he could not get a new release, he decided to redisclose my protected medical records anyway to another doctor for an expert opinion without my knowledge or permission and without ANY release. The new physician did read the records and sent a written report back to this attorney, all without a signed release from me. Could you tell me what my options are for restitution? Could I bring criminal charges or does the state or federal gov. do that? How should I proceed?

You can report this to the Supreme Court Commission.

I assume you have an attorney who is handling your affairs? My guess is that you should listen to what they have to say.
