Parental rights

If a person is paying child support and covering a child with insurance and there is no court order stating custody of any sort or visitation, can the parent whom the child lives with keep the other parent from seeing the child?

Yes. That is what is known as a fait accompli. If you want visitation or custody there's got to be a court order.
