Legality of Changing a revocable Trust

If a person declared her wishes in the presence of her two daughters and grandson with a lawyer taking notes in order to prepare a revocable trust, can one of the daughters legally take over everything given the Mother's dementia and without the knowlegde of the other two by having the Mother sign everything over to her. Is the Mother's signature valid?

Not if she lacks capacity. If the mother didn't know what she was doing or what she was signing, it's not valid.
