Denied clain for surgery

I was injurred 11/30/2000 on the job. I fell from a ladder and twisted my knee, ankle and landed on my buttocks jarring my back. My Orthopedic Dr. says that I need surgery on my knee and workmans comp. is denying me. At first they said that the knee wasn't listed as an injurred body part on initial claim, which is untrue and I sent them a copy of original report to prove it. Now they offer no explanation only that they will not pay. I work 11 hour shifts and do a lot of lifting and climbing, i cannot continue to so this without help. My knee swells twice it's normal size and shoots pain up my leg after 5 or 6 hours on it at nite. Please give me some advice.
