When an accident occurs and someone is driving your car and the PIP from your car insurance has been exhausted; who is responsible for any pending bills?

This question cannot really be answered without knowing details about the accident. Responsibility for damages depends on who was negligent (that is, who was at fault). PIP coverage does not involve any determination of fault.

Depending on where it was issued, the driver's PIP may provide secondary coverage after exhaustion of the PIP on the occupied vehicle. Also, in MD a claimant can use his health insurance benefits, although they will expect reimbursement if there is a subsequent recovery from the liability insurance of the at-fault party.
