how do i change my last name
im 16 and i want to change my last name how do i do it?

You are not yet old enough to petition without parental assistance. You should discuss this with your parents.
When you have parental assistance, or are 18, you petition using a form providing by the Probate and Family Court. A judge will almost always allow the petition.
