Child support and Peranting time issues

i cant find the paper work or what it is called to start a case for child support and parenting time i need to know how i can do this pro per for i am indegent and would love to see my one year old daughter ive been to FOC and ive been to FIA and nothing no-one will help or tell me what to do or what papers i need to start my case i can see why so many dads are not wanting to go through with this headache please can someone help me...i do have a sworn affidavit from my ex and i that i am the father

You should be able to go to your county's Friend of the Court and ask for the Motion form. You will be filing a Motion for Parenting Time, etc. and the form is self-explanatory. Find someone who will help you. The process should fairly simple. Good luck!
