Affidavit of service

I am submitting a "notice of appearance and written plea of not guilty" form to avoid appearing in person for an arraignment. I am required to attach an "affidavit of service" form to my written plea of not guilty form. I obtained an "affidavit of service" form but I am not exactly sure what information is required on the form or who is suppose to serve this form. Any data would help me to clear up my confusion! Thankyou

Apparently you do not have a lawyer. You should get one. If you have no job, apply for public defender representation. Who told you were required to attach an affidavit of service to the arraignment waiver in a misdemeanor case? I've never seen or heard of that being required. The purpose of an affidavit of service is to ensure the opposing party (here, prosecutor) has notice. Try the "find forms" feature of this web site -- search for a completed affidavit of service. Normally it will require the state and county, the name of the person served if personal service, and the name and address if service by mail. There are different forms available for personal service and service by mail. If it is served by mail, the person mailing it also fills in their name, swearing that they did mail it, postage paid, on a certain date. A clerk of court could explain it in person to you.
