How do I fill out a subpoena?

I have a subpoena from Ramsey County. I want to subpoena my sons fathers checking and savings account statements for the past year so that we can review those for our child support hearing in February. I am only subpoenaing documents, directly from the father. How do I fill out this form? I marked the box that says, ''duces tecum''. There is an area on there that says ''witness'', who is that? Is that the person I am giving the subpoena to? Any help would be appreciated. thanks

Yes, the witness is the person you want the documents from.
