do you need written approval from the ex-spouse if you are going on a trip out of the state?

No, unless there is something in your custody order to the contrary.

Typically, no you do not but look to the Order to see. If you are free to travel out of state with your children, consider giving your ex-spouse contact information for while you are traveling.
