i fell down at a smiths grocery store the floor was wet i am 100% disabled i have kidney disease, muscle neuropathy, and have type two diabetis . smiths sent me a letter wanting to know all my doctors for the last 10 years and the medications i am on . they also want to know if i have HIV, my mental state and any alchol abuse is this normal
