If you are currently in the US in H-4 status, then you will not have to leave in order to obtain H-1B status. However, in order to travel, you will have to obtain an H-1B visa prior to re-entering the US. The questions vary depending on the Consulate. Applicants have to fill out form DS-156 (and DS-157 if they are male). An attorney will be able to futher assist you with the H-1B process and the visa application.

I would need more information regarding your particular case before I could comment on it properly.

However, what I can say is that any questions that may be asked at an interview will be based on the information in your file and what the immigration services discovered during the investigation they conducted.
