Legal Question in Intellectual Property in New York

At the moment Im filling a Trademark paper form (I like the old fashion way) how ever I still have some issues.

1) Under the tag Applicant Entity information where it says name(s) of general partner

I want to state my partner as limited in that section, does that mean I should put my name also along side with their name.

2) Where it state name under the tag Applicant information should my limited partner full name appear along side my name.

Any business or franchise attorney will say to follow the instructions on the form. It doesn't say anything about disclosing your limited partner. And usually limited partners don't want their identity disclosed. Follow the instructions and consult with a good business or franchise attorney in your area for specific advice.
