mental incompetence

i have schizophrenia. i have a power of attorney .i have been found incompetent and can not sign for anything i must have another person sign for me. can this be the responsibility of my POA or is there another document i need to fill out?

Providing the Power of Attorney was durable (continues during incapacity), it should be sufficient to allow the holder to sign things for you.
