My wife has our house in her and her husband's name from her 2nd marriage. What happens if she dies?

I assume you are her thrid marriage. If this is correct, and assuming that your wife's deed says "husband and wife", the wife and second husband become tenants in common and your wife's share passes through her will (or intestate if she does not have a will). It would not go to her ex husband.
