name change back to name before adopted

My nephew would like to change his name back to his original name and birth certificate before he was adopted. How can he do this - he is an adult.

As long as he knows what name he wants to change it to, he can file an action for a name change in Family Court. The process is not terribly difficult, though there are some "hoops" that must be navigated before the name change can be granted.

A name change is accomplished by filing an action in family court. It is not too complicated but you must have a hearing and certify that you are not doing it to avoid debts or criminal prosecution in your current name.
