Legal Question in Real Estate Law in Tennessee

Hi, my wife and I currently take care of her mother in a home I acquired and have being paying for. It has become necessary to place her in a home with professional care. The home is in her name because of a better interest rate at the time.We need to have it in our name before we can sell it and move closer to family.My wife has power of attorney and I could pay it off if necessary.We want to do this in a timely manner because her condition worsens daily.(she's pushing 90 )What would a bank require (paperwork, etc) because with her bedridden we cant get her out.Thanks if this makes any sense
