Legal Question in Family Law in Texas

I'm in Texas. If I'm undecided about whether to change my name back to my maiden name, should I put a name change provision in my Final Decree? If I do, is the change optional? If not, how soon must I file all the necessary papers (DMV, SSA, etc.)?

Include it now. Otherwise it's expensive and time consuming to do it later.

You can do the actual change later when you feel like it.

If you decide not to go through with it, you don't have to but include the request in the Final Decree of Divorce - it's free and easy now.
