Legal Question in Immigration Law in Virginia

Affidavit of support

I applied an immigrant visa for my MOM, NVC asked me to fill up the affidavit of support form and send to NVC. Question is there are no of affidavite of support forms, I need to know what perticular form I need to fill up.

Here following is the list of forms

1.I-864W

2.I-864P

3.I-864EZ

4.I-864A

5.I-864

Please let me know, i will appriciate it.

I-864P(if intended for parent of sponsor) should work. Otherwise, regular I-864.
