what forms do i need to file with the courts to take a first deed of trust on a quite title to count to have it discharge

Your inquiry is confusing. If you're referring to filing an action in the circuit court to "quiet title" to a piece of property which is the subject of a first deed of trust which you referenced, you'll need more than a set of forms to accomplish this objective. You'll need an attorney who is appropriately knowledgeable and experienced in these kinds of cases

as this is not a matter for a so-called "proseer", meaning one who

files a case on his own and represents himself in court without the aid of counsel.
