survivors benifits

My mom died when i was 2 yrs old and since then ive recieved survivors benifits. Theyve always been in my name c/o my aunt or dad. I was kicked out of my aunts house and moved to CA and live with my sister. My dad is still getting the checks. he gets them and then writes my sister a personal check for the same amount. However im moving out on my own in 3 months, and im going to need that benifits check sent in MY name to MY house, not my fathers. But he will not change the name and refeuses to put the check in my name. Im turning 18 in october, and i know at that point he cant stop the s.s. check being changed to my name. BUT my question is how do i do that? Ill still be going to HS and i know that going to school is a requirement, so that shouldnt be a problem. But how do i switch the name to no longer be my dads, and to make it solely my name on the check? I NEED to know this i depend almost completely on that check for my survival, escpecially since my dad doesnt give me financial support. and just even missing one month of bemifits will harm me drastically. so if you can please tell me how to switch it, and maybe tell me where i can get forms, or whatever, that would be VERY helpful.
