question on missing i-94 with h1b extension approval

hi

if a person who is on the 3rd yr of h1b applies for a extension and gets an approval without a new i-94,

what could be the possible reasons

he had been continuosly working during these 3 yrs and never been out of status....... but still

why he didnt get a new i-94

he converted from f1 to h1 3 yrs back and got a i-94 at that time and never left US since then for

get his h1b stamping

please advise

That is very odd!!! You need to go for stamping but get a new I-94 before you leave USA.
