Legal Question in Entertainment Law in Arizona

I am writing a book that will require people to submit their stories to me. Once I l select the best ones I need a release form of some sort to let the people know that they will NOT receive any type of royalties once the book is sold or if it is ever made into a movie. Not sure of what type of legal form I would need or where to get one. :)
