Childs request to change primary residence

What do I have to do/what docs or format do I use inorder to petition the court to have my ten year old daughters wishes granted to reside with me primarley again. The first six years she was with me the last four with father. After three years with him, new step mom and adopted sister. She is still not getting along. Father had attempted to limit my rights in 1998 due to my current partners and father of two other children backgroud and failed. I was granted a custdy evaluation and our existing plan was inforced, Jointlegal custody with prim physical with him. The evaluator assured my ten year old that she would be able to live with me again - in forth grade.

You must file a "Petition to Modify Custody". Therein, you have to show, in writing, how circumstances have changed since the entry of the current orders, and then show why it is in the child's best interests that a change be made.
