There is not form for terminating supervised or any other visitation.

To change visitation a judge must sign an order changing visitation. You need to file an Order to Show Cause requesting a change in visitation. You will need evidence that visitation with your daughter is detrimental to her. That is there is a high probability of her being injured physically opr emotionally if there is continued contact.
