relinquish of parental rights

Is there a proper form for my ex to fill out to relinquish his parental rights of our two children so my new husband may adopt?

I reside in CA

Has your x agreed to it? If so then an affidavit to that effect may be drafted, or you may start the adoption process and if there is no objection, it will be accomplished. Call me directly or e-mail me for assistance.
