How do I supeona a witness for a child custody case. Do you have a sample document?

Re: how to supeona a witness

The clerk of the court will provide you with a subpoena that has been issued. The subpoena is then filled out and served on the pesron to be subpoenaed. The subpoena must be served by a person that is over 18 and is not a party to the lawsuit.
