You need an Order to Show Cause and an Application and Supporting Declaration. Depending on the county you may need a mediation appointment form. You file these and serve them and then you need a proof of service. The Family Law Facilitator can help you prepare these forms or you can look for a free or low cost program. Try the county bar association. some people use paralegals for form prep as well. This is really not giving legal advice.
