Indie contractor needs her tax forms

My contract was terminated prematurely for reasons never revealed to me by my supervisor/person who hired me. I was an independently contracted mental health consultant. She has also not provided me with my tax forms for the past year's income. I called her about 10 days ago for it, but still nothing has arrived nor has she returned my call. Is this legally actionable? What should I do- put my earnings check stubs together and figure out what I owe on my own- or hand ot over to my tax man?
