How do I file a lawsuit without a lawyer?

I have an issue I am reaching 1 year of my auto accident and I still have therapy left. The insurance company will not respond and no one is taking my case. I want to file a law suit to protect my interest, but I don't know were to begin. I need to file in the La court house what forms do I need to start. Any advise?

Any advice? Yes, consult with a law firm that specializes in Personal Injury matters. What is the value of your injury claim? You need to know this to know which court to file in. Of course the insurance company isn't talking to you. The closer you get to the one year statute the less incentive an insurance company has to speak to you. They want to give you the opportunity to miss the filing date. They know you are under pressure to settle the case before the year is up.
