Name change to maiden name

I was divorced in Clay County Florida over 16 years ago and kept my married name. I have no idea (for over 16 years) as to the whereabouts of my ex husband but would like to change my name BACK to my maiden name. There were no specifications or mention of this either way in my divorce decree. What are the procedures? Thank you,--name removed--

You may obtain a form, Petition For Change of Name, at your county Clerk of Courts office or from the Florida Supreme Court or other websites which provide forms. Complete the petition and a proposed Order form, file it with the required fee at the Clerk of Courts. Ask the Clerk's office about scheduling a hearing before the court and other local procedures followed for such petitions. It is a relatively simple procedure.
