Childrens medical release while out of town

My husband and I will be going out of town for one week. We are leaving our 2 children with family and close friends. What sort of release do we need for them in case the children should need medical care? I know that in the case of a life threatening emergency that a release is not necessary. I need something in case of illness or broken bones etc.I have notified our pediatrician and dentist but would feel better with something legal in writting.

You would need a document signed by both parents stating who would be permitted to make the decisions concerning the children. Those decisions would be those set by you. This document would be signed and notarized and given to those who will be caring for the children. You could even put an expiration date on this document. Very common document.
