funds in county registry

i have funds in county registry and am attempting to get the correct form for a hearing for disbursement of funds to a petetioner,, what foms am i to use along with form for judges order?

Normally the county clerk can tell you if you even need a form. If you do, then you won't be expected to be very formal about it. Give the judge a motion and a separte order to sign. If the court won't accept your motion, then an attorney won't charge much to give you the proper form.
