child custody

my 5 yr old son has lived with me for the last 6months and his mom is in the army and school is starting in 1 week and my name is not on birth certificate and it is difficult to get him registered in elementary. the assistant principal said i needed a power of attorney & the vital records said i needed a paternity acknowledement form, can you please assist me in what to do?
