Legal Question in Credit and Debt Law in Illinois

Hello, I will be filing chapter 7 in a few weeks but I have an intent to assign wages and will be submitting a Revocation of wage assignment. I need to know if I need to fill out the Affidavit of Defense? I live in Illinois and don't really want to keep the vehicle.
