procedure for taking minor child out of state. ex has given permission

My current husband has accepted a job out of state. My ex has given oral permission, but i need to know what procedures are necessary for making it "legal". Is there a form somewhere? Do I need to get a letter from an attorney? I only have three weeks, and I would like to write my own letter for him to sign, and file it myself.

Hello. The right way to

do this, in the best interest

of your child and you, is to

file a petition to remove

your child out of state and

in the petition set forth

all details. With a court

order granting removal, you

will be safe.
