Depends upon the situation; there are no hard and fast rules for this, though ordinarily we insist upon a certified copy of the FIR for filing anticipatory bail petition; but in exceptional situations, a case can be filed without FIR.

You can apply. But it may be futile. One of the essential requirements is reasonable apprehension of arrest in a non-bailable offence. FIR/Crime Number provide answer to said essential requirement.

in case u aware that f.i.r was lodged at a particular police station, still u may file an application for anticipatory stating the location of police station and the concerned official from the police station would disclose whether any case is pending against you or not.
