Divorcee wants name changed back to maiden

I did not return to my maiden name when I got divorced five years ago. I now want to do that, how do I get the forms I need and file for this in my city, and also, how will this affect the chapter 13 I just filed?

You can file a Petition to Change Name in the county where you live. You will have to pay a $104 filing fee, and you will have to pay for an ad in a general circulation newspaper, announcing your name change. The ad must run three times and then be submitted to the court.

Then you will have a hearing at which the judge will grant your name change and give you a Decree of Name Change.

You can do all this yourself, if you don't mind reading up on the subject and doing some legwork. Otherwise, it's not a complicated thing, and an attorney shouldn't charge you much to do it.

I cannot think of any way that it will affect your Chapter 13 that's already in progres.
