can a family member other than the purchaser of a deceased burial plot place a headstone on the deceased grave, when the deceased has no headstone after 20 years?

That depends on the type of plot, and you need to talk to the cemetery to know that fact. Unless it is a family plot, I know of no reason why a person could not buy a headstone for a grave.
