Need Help With Garnishment on Judgment

Divorced in 1999. Court ordered ex-husband to pay to me ''half'' of 401(k) that ex-husband had cashed in and spent. February 2000 Judgment was entered, ex-husband found guilty of contempt for not following court order. I am Pro Se (no $ for lawyers fees) and have not had much luck in finding garnishment forms needed to submit to the court. Where will I find such forms? Any and all help is appreciated!

In Kentucky, the Circuit Clerk should have the forms. They are multipart.
