What is in a name?

When I was born the name on my Birth Certificate is First: Julie Middle: Anne.

My mother wanted to name me--name removed-- One word. So when they filed for my SS Card I was--name removed-- my pediatrician:--name removed--in fact EVERY record that has my name on it is--name removed--M.--name removed--

I am filing for a grant that requires my birth certificate, medical records and SS Card. Due to the fact that NONE of these match my Birth Certificate they want me to re-apply under the name Julie Anne.

Do I have to get my name legally changed to keep this from happening again?
