Ways to bypass creditors when transferring your home as part of your estate

A 63-year old married, father of four living in Massachusetts wants to know how he can protect his home from creditors in the future if he is put in a nursing home later in life. How can he prevent the nursing home from taking his house if he goes into a nursing home eventually? What is the best way to transfer his house as part of his estate in order to avoid taxes? Should he transfer the house to his wife's name or his children's names? Is a trust the best way to go? He wants to make this as simple as possible, but his major concern is protecting his house from the nursing homes.
