Legal Question in DUI Law in Michigan

To whom it may concern,

I recently had a Divers Lcense Apeal Hearing and my appeal was deined.

As indicated on the "order", I am allowed to appeal this ruling to the

Circuit Court (of Oakland County - Michigan). What is the process that I follow for this Circuit Court appeal (forms, written request, etc.)? I am not being represented by an attorney at this time nor can I afford one.
