Legal Question in Family Law in Minnesota

I filed a motion in MN (Martin Co jurisdiction) for custody of my 17yo son. He lives with me in AZ, ex sent son to live with me in June 2013. It has been 6 months and I want to dismiss my MN case and open one in AZ. There has been no action on my MN motion (we were ordered to mediation and the motion was reserved). What form do I file to dismiss my motion?
