Pay Raise % Printed on review, but only half was Given

I have a hardcopy of one of my reviews from work. On the last page is a formula that states ''Increase Calculator: Current Rate X % of Increase = Increase Amount'' and then a box that states ''New Rate''. In the % of Increase box my employer has 5%, however the Increase Amount figure works out to 2.5%.

I talked with my supervisor and he told me I was misinterpreting the form. He stated that I would be given 2.5% and then the other 2.5% at my next 6 month review. However, I asked him if that was the case, then why would I get another review to determine changes to my hourly rate? I asked if I was promised the other 2.5% at that 6 month mark and was told no because they would give me another review to see if I earned a rate increase.

I am confused. I have a hardcopy that states I earned a 5% increase in hourly wage, how can they only pay me a 2.5% increase?

I would need to see the form, especially in a situation where the only dispute is correct interpretation of the form.
